Permission to record therapy sessions.
Recorded therapy sessions allows for both the therapist and parent to reflect on progress made in therapy as well as to set new treatment goals as an ongoing process.
With this form you as the parent give Riana Henning, Occupational Therapist, permission to record therapy sessions and to store it in a safe way by encrypting it, thus only the person with the password can access the video.
These recordings will only be used by Riana Henning herself to review sessions and to plan and adjust treatment.
These recordings can also be used during consultations by Riana Henning with parents to discuss progress and future therapy.
Riana Henning is also an Expert Training Leader for the DIR Floortime® Training Program and First Last teaches for the Interdisciplinary Council on Development and Learning Disorders (ICDL) on a regular basis.
Due to the training she uses video material to demonstrate certain aspects of DIR/Floortime.
Riana Henning would therefore like to ask for permission to use the recorded video sessions for teaching purposes.
If you will permit me to do so, please complete the following below.
